مقدمه:پیلونفریت یکی از شایعترین بیماریهای کودکان میباشد. درمان به موقع با آنتیبیوتیک مناسب نقش مهمی در بهبود بیماری و جلوگیری از پیشرفت و عوارض دراز مدت آن دارد. درمان تجربی پیلونفریت نیازمند شناسایی شایعترین میکروارگانیسمهای ایجادکننده و الگوی مقاومت آنتی بیوتیکی آنها میباشد. با توجه به افزایش مقاومت به آنتیبیوتیکهای موجود و تغییر الگوی مقاومت آنها با گذشت زمان و در مکانهای مختلف، این مطالعه با هدف بررسی شیوع مقاومت آنتیبیوتیکی میکروارگانیسمهای شایع دستگاه ادراری در کودکان بستری با تشخیص پیلونفریت حاد در شهر کرمان از فروردین ماه سال 1393 تا پایان اسفند ماه سال 1394 انجام شده است. شیوه مطالعه: در این مطالعه توصیفی-مقطعی 181 کودک بستری با تشخیص پیلونفریت حاد مورد بررسی قرار گرفتند. نمونهگیری در کودکان بدون اختیار به وسیله کاتتر ادراری و در کودکان دارای اختیار به روش نمونه وسط ادرار انجام شد. آنتیبیوگرام کشتهای مثبت به روش دیسک دیفیوژن برای تعیین مقاومت آنتیبیوتیکی میکروارگانیسمها انجام گردید. یافتهها: در این مطالعه شایعترین ارگانیسم، اشریشیاکلی با فراوانی 72.6% بوده است. این میکروارگانیسم بیشترین مقاومت را در برابر آنتیبیوتیکهای خوراکی کوتریموکسازول (46.6%) و سفالکسین (46.6%) و کمترین مقاومت را در برابر آنتیبیوتیک خوراکی نیتروفورانتوئین (2.2%) و آنتیبیوتیکهای وریدی سفتریاکسون (3.4%) و آمیکاسین (3.7%) نشان داد. نتیجهگیری: با توجه به یافتههای این مقاله پیشنهاد میشود در شهر کرمان در مواردی که لازم است، ادامه درمان پیلونفریت به صورت سرپایی انجام شود و از آنتیبیوتیکهای خوراکی کوتریموکسازول و سفالکسین به دلیل مقاومت بالا استفاده نشود. با توجه به تمایل بیشتر به مصرف آنتیبیوتیکهای تزریقی سفتریاکسون و آمیکاسین به علت مقاومت کمتر آنها، توصیه به تغییر دورهای استفاده از این داروها در درمان پیلونفریت حاد میشود تا از افزایش مقاومت آنتیبیوتیکی آنها جلوگیری شود.
Background: Pyelonephritis is one of the most common pediatric diseases. Early treatment with appropriate antibiotics plays an important role in improving the disease and preventing its progression and long-term complications. Experimental treatment of pyelonephritis requires identification of the most common involved microorganisms and their antibiotic resistance patterns. Due to the increase of antibiotic resistance and changes of resistance pattern in different places and over time, this study was performed to investigate the prevalence of antibiotic resistance against common microorganisms of urinary tract in children with acute pyelonephritis hospitalized in Afzalipour Hospital of Kerman/ Iran during 2014 to 2015. Methods: This descriptive and cross-sectional study was performed on 181 children with acute pyelonephritis who were hospitalized in Afzalipour Hospital of Kerman during 2014 to 2015. Urine samples were collected from children with urinary incontinence using urine catheter and from children without urinary incontinence through mid stream clean-catch method. Then, the antibiogram test was performed on positive cultures using disk diffusion method to determine the antibiotic resistance. Results: Escherichia coli was the most prevalent microorganism (72.6%). This microorganism showed the highest resistance to Co-trimoxazole (46.6%) and Cephalexin (46.6%) and lowest resistance to oral Nitrofurantoin (2.2%) and IV Ceftriaxone (3.4%) and Amikacin (3.7%). Conclusion: It is suggested that in outpatient treatment of patients with pyelonephritis in Kerman region, avoid using oral Cotrimoxazole and Cephalexin due to their high resistance. Since there is high tendency to use IV Ceftriaxone and Amikasin, it is recommended to change these antibiotics periodically in order to prevent antibiotic resistance.
